In Texas, the certifying body is the Texas Department of State Health Services (TDSHS), and the certification exam is administered through NREMT. Paramedic is the full scope of prehospital care. Training requires 1,200 to 1,800 total hours and takes one to two years to complete. The NREMT Paramedic exam is a CAT with 80 to 150 questions, a 2.5-hour time limit, and a $175 fee per attempt. The national median paramedic salary is $58,410, with the top 10% earning over $82,420 according to May 2024 BLS data.
In Texas, the distinction between EMT-Paramedic and Licensed Paramedic matters. To earn a Licensed Paramedic credential from TDSHS, you need either a two-year EMS degree or a four-year degree in any field. The Licensed Paramedic designation is what most career fire departments and progressive EMS agencies require for senior positions.
Licensed Paramedic (LP) in Texas is the highest prehospital certification level. It carries legal authority and clinical scope that the basic Paramedic certification does not, and it is increasingly the minimum standard for career advancement in Texas EMS and fire departments.
Why the AEMT Step Matters
Many candidates look at the career ladder and see three jumps: EMT to AEMT to Paramedic. Then they decide to skip AEMT and go straight from EMT to Paramedic school. This works for some people, but there are strong reasons to consider the intermediate step.
First, AEMT gives you clinical experience with invasive procedures before you commit to a 1,200-hour paramedic program. Starting IVs, managing airways with supraglottic devices, and pushing medications in the field builds confidence and competence that makes paramedic school significantly easier.
Second, the pay bump is real. Moving from EMT-Basic ($41,340 median) to AEMT ($45,000-$50,000) while you are saving for paramedic school or waiting for a program seat makes financial sense. You earn more while you prepare for the next step.
Third, some paramedic programs give credit for AEMT coursework, reducing your total training time. Check with your target program before assuming this, but it is worth asking.

Fire departments amplify this further. A firefighter/paramedic in Texas earns significantly more than a standalone paramedic because fire department pay scales, overtime structures, and pension benefits are built into the compensation. Dallas Fire-Rescue adds $3,000 to $5,000 per year for paramedic certification on top of base firefighter pay. Houston FD pays Step 2 firefighters $58,738 after academy completion, with senior firefighter/paramedics exceeding $85,000.
